Required Forms (Turn in At Band Camp)

These forms must be turned in at band camp. The Medical Form MUST BE NOTARIZED. There are notary signers at most Banks, Lawyers Offices, as well as the LGHS Front Office. These forms go with us on every trip!

SPORTS PHYSICALS FOR BAND ARE NOW REQUIRED. Here is the information.
Sports physicals are now being required for all Polk County Public Schools high school band students. Please follow these steps to complete this process:
- Create an account on the website/platform "Aktivate"
- Please see the attached instructions for how to create an account.
- Complete the Sudden Cardiac Arrest Course
- Complete the Heat Illness Prevention Course
- Complete a Sports Physical (this can be done with your students primary care physician, urgent care, etc.)
- Upload your Sports Physical to your Aktivate Account
- DO NOT turn your physical into Ms. Lutz, it MUST be uploaded to Aktivate

WHAT TO WEAR--- (posted 3.25.25)
WE WILL BE OUTSIDE, IT IS GOING TO BE HOT, & WE ARE GOING TO BE MOVING!! :D For their safety, students are NOT permitted to wear jackets, hoodies, sandals, crocs, or jeans while outside.
We recommend:
- School appropriate tank tops (no spaghetti strap, no low cut, no crop tops)
- Light colored, dri-fit, school appropriate t-shirts
- School appropriate shorts or leggings
- Required: Closed toe sneakers (something easy to move in such as tennis shoes, not boots)
WHAT TO BRING (Required)-- (posted 3.25.25)
WE WILL BE OUTSIDE, IT IS GOING TO BE HOT, & WE ARE GOING TO BE MOVING!! :D
- YOUR FIELD TRIP & MEDICAL FORM (complete the "Student Info Form" online)
- Instrument/Guard Equipment
- Your own personal 2 quart cooler of water (there are no water fountains available outside)
- A bagged lunch
- Students may NOT have food delivered to the school, they must bring it with them
- There is not a fridge or microwave available for student use
- You MUST eat lunch, your body needs the nutrients to replenish yourself after being outside!
- Winds (brass/woodwinds)- Flip Folder and lyre (available at Carlton Music, or Guitar Center)
- This is not optional & will help students greatly if they have them for band camp!
- Winds and Percussion- A Pencil
- Percussion- A Three Ring Binder with plastic sheet protectors
HOW TO CHECK IN--- (posted 3.25.25)
The check in table will be at the main entrance of the music building (facing the parking lot). Arrive early enough to turn in your forms.

- Officer Work Day: Band Room
- Everyone: Start in the Band Room
- Inside Portion: Winds in Band Room, Percussion in Chorus Room, and Guard in Auditorium
- Outside Portion: Back Practice Field
BAND CAMP SPIRIT DAYS!--(posted 3.25.25)
It's always more fun to see students showing off some spirit, so join us in the following dress up days!
Tuesday 7.22.25-- Misfit Day! (wear mismatched clothes!)
Wednesday 7.23.25-- Twin Day!
Thursday 7.24.25-- Beach Day! (dress for a school appropriate beach vacation!)
Monday 7.28.25-- Your Type!
Tuesday 7.29.25-- Cops & Robbers!
Wednesday 7.30.25-- Fave Artists/Band Tees!
Thursday 7.31.25-- Rhyme Without Reason! (pick a person to coordinate with, for example, FBI and Butterfly)
BAND CAMP ACTIVITIES!--(posted 3.25.25)
"All work & no play makes Jack a dull boy." Each day we will have an activity planned for students to participate in!
Tuesday 7.22.25-- Duck, Duck, Goose & Kona Ice!
- We will play a full band game of Duck, Duck, Goose & students can bring money to purchase Kona Ice afterwards!
- Students will play a game where they have to ask questions to figure out what item is listed on their back!
- We will have water balloons, water squirters, etc.
- Please bring a towel & a change of clothes!
- Which team can conquer all the clues to find the prize at the end?
- Band sections will work together to create a flag to represent their section, this flag will hang some where in the music building!
- Sections will face off in a game of kick ball!
REHEARSAL TIMES, SHOE/UNIFORM FITTING--(posted 3.25.25)
OFFICER WORK DAY
Monday July 21st: 8:00 am- 4:00 pm
FULL BAND CAMP---- ALL MEMBERS, NEW AND RETURNING WILL COME TO BAND CAMP
Tuesday July 22nd- Thursday July 24th: 8:00 am-4:00 pm
Monday July 28th- Thursday July 31st: 8:00 am- 4:00 pm
PARENT PREVIEW PERFORMANCE
Tuesday August 5th: 7:15 pm (Students are expected to be in the band room by 6 pm to rehearse for this performance)
This will be the first performance of the year to show off what we have learned during camp so please come out to our back practice field (behind the school) to support our Tribe Members!
PRE-SCHOOL EVENING PRACTICES
Tuesday August 5th: 6 pm-8 pm
Thursday August 7th: 6 pm-8 pm
SHOE FITTING, Thursday, July 31st @ 10:00 am
All students are required to have a pair of marching band shoes (dinkles). New students, or students needing new marching shoes, the shoe company will be here during camp for shoe fitting. Winds and Percussion Shoes will be around $36 and Guard Shoes will be around $49. Checks/Money Orders need to be made out to LGHS Band, or Cash can be accepted as well. THIS IS NOT INCLUDED IN YOUR DUES!
UNIFORM FITTING, DATE TBA
Winds and percussion students will be having their marching uniform fitting take place one day during band camp. Students will need to wear a t-shirt and non-bulky shorts to try on the uniforms to ensure a proper fit. Time blocks are going to be sorted by grade level.
BAND DUES
Between performing at football games, parades, MPA’s, etc. our band students do a lot! Because of this we incur a lot of expenses. To participate in the band program students are required to pay band dues. You will have the opportunity to decrease dues through volunteering & fundraising. Please see the chart below detailing what their dues cover.
