These forms must be filled out as soon as possible and turned in with the Student Information Form. The Medical Form MUST BE NOTARIZED. There are notary signers at most Banks, Lawyers Offices, as well as the LGHS Front Office. These forms go with us on every field trip.

SPORTS PHYSICALS FOR BAND ARE NOW REQUIRED. Here is the information.
Sports physicals are now being required for all Polk County Public Schools high school band students. Below are the forms that you will need to create and account on the website/platform "Akitvate", which all of your course work certificates and physical evaluation forms will be uploaded, and how to create an account on NFHS, which is the website/platform where the online course work has to be completed for the certificates.

Guard- Athletic Clothing that is school appropriate at will allow for ease of movement. Also, close toed closed back shoes.
Winds/Percussion- Shorts and T-shirt that is school appropriate. Also, close toed close back shoes

Monday 7-29-24-- Mr. Luce Monday, (Khaki shorts, polo/lg band t-shirt, sneakers)

WHAT TO BRING-- Updated list (posted 5-1-24)
-YOUR FORMS,
-Instrument/Guard Equipment
-Your own personal 2 quart cooler of water. There will be NO SHARING OF WATER and WATER FOUNTAINS WILL BE TURNED OFF
-A Bagged Lunch/dinner. You must eat to get nutrients in your body during camp, AND, there will be no sharing of food with others.
-Winds (brass/woodwinds)- Flip Folder and lyre (available at Carlton Music, or Guitar Center)
-Brass Players- a spit valve rag (to empty your spit valves onto, preferably an old towel)
-Winds and Percussion- A Pencil
-Percussion- A Three Ring Binder with plastic sheet protectors

-The check In Table will be at the main entrance of the music building (facing the parking lot). Arrive early enough to turn in your forms.

This year we will not be having a separate Rookie, Guard, Percussion, Officer Camp. We will All State Together
FULL BAND CAMP---- ALL MEMBERS, NEW AND RETURNING WILL COME TO BAND CAMP
Wednesday July 24- Thursday July 25, AND Monday July 29- Thursday August 1, 8:00 am- 4:00 pm
PARENT PREVIEW PERFORMANCE
Tuesday August 6, Time- TBA
This will be the first performance of the year to show off what we have learned during camp
PRE-SCHOOL EVENING PRACTICES
Tuesday August 6, and Thursday August 8, 6:30 pm- 8:30 pm.

SHOE FITTING, DATE TUESDAY, JULY 30, 2024- Students needing new marching shoes, the shoe company will be here on Tuesday, July 30, 2024, at 10:00 am. for shoe fitting. Winds and Percussion Shoes will be $36 and Guard Shoes will be $49. Checks/Money Orders need to be made out to LGHS Band, or Cash can be accepted as well.
UNIFORM FITTING, DATE TBA (updated 7-25-24) Winds and percussion students will be having their uniform fitting take place on TBA. Students will need to wear a t-shirt and non-bulky shorts to try on the uniforms to ensure a proper fit. Time blocks are going to be sorted by grade level and are as follows
Seniors- Monday, 7-29-24, 7:00 am- 8:00 am
Juniors- Tuesday, 7-30-24, 7:00 am- 8:00 am
Sophomores- Wednesday, 7-31-24, 7:00 am- 8:00 am
Freshmen- Thursday, 8-1-24, 7:00 am- 8:00 am
PARENT PERFORMANCE NIGHT, TUESDAY 8-6-24- The Tribe will be putting on a "Parent Preview Performance" on Tuesday, 8-6-24. The performance will begin at TBA pm in the courtyard behind the music building. Come see and hear what all we have learned over the past week of camp!!!!

UNIFORMS, MARCHING SHOES, AND DUES--- (posted 5-1-24)
UNIFORMS-We will be wearing our marching uniforms at some point in time during the season so there will be a uniform fitting day. More information about that will be given later
MARCHING SHOES- Marching shoes will be a part of the uniform and must be purchased by the student. These are not exact prices but the shoes for winds/percussion are around $30-$35, and the guard shoes are around $40-$45. Exact shoe prices will be given during band camp.
DUES- Since marching band is back to "normal" for the second year in a row. There will be band dues again this year. A Breakdown list of what the dues cover, the amount, due dates, and ways to "work down" dues will be given out during band camp.
